We work with individuals that have been assessed via a MN Choice Assessment and approved for Brain Injury (BI) ,Community Alternative Care (CAC),Community Access for Disability Inclusion (CADI),Developmental Disabilities (DD),Alternative Care (AC),Essential Community Supports (ECS) and Elderly Waiver (EW).
