This free, comprehensive assessment helps determine what kinds of supports and programs you may qualify for — including waiver services like Elderly Waiver (EW), Alternative Care (AC), or Essential Community Supports (ECS).

What Is a MnCHOICES Assessment?
The MnCHOICES Assessment is a statewide tool used by Minnesota counties to evaluate a person’s needs for long-term services and supports.It’s available to:
Adults and seniors who may need help living independently
Individuals with disabilities
Caregivers seeking resources or relief
During the assessment, a county or tribal assessor meets with you (and anyone you want present) to discuss:
Your daily living needs (such as cooking, cleaning, bathing, mobility)
Your health conditions or physical limitations
Your living situation and support system

Step 1: Contact Your County or Tribe
To begin, contact your local county’s Human Services office or tribal nation’s health and human services department to request a MnCHOICES Assessment.

Your name and date of birth
Address and contact details

Step 2: Schedule Your Assessment
